Company may update available addresses via the Company/Entity Address Portal.  Link is located on the IDOI website at IDOI: Company/Entity Financial Compliance under the Resources section.  If individual making the updates doesn’t have the required PIN# they may select the “Need PIN?” link to display the current PIN contact.  If PIN contact isn’t established or needs to be updated a request can be submitted to fnclsvcs@idoi.in.gov .

*NV No   Nevada requires all licensed companies to make contact/addresses (excluding statutory home office address)/email address updates through the Nevada Company Address Change Service Portal on the Nevada Division of Insurance webpage at https://doi.nv.gov/Insurers. Contact state for requirements and pin number at finances@doi.nv.gov. Statutory home office address changes require a UCAA Corporate Amendment Application.

NY Yes   The Life Bureau prefers both the UCAA electronic submission and hard copies, especially for large files that are easily readable in hard copy format.  The Property Bureau requires the submission of  two hard copies in addition to the UCAA electronic submission.  The Health Bureau requests that hard copies  be mailed to the Health Bureau and email copies be sent to: HealthBureauRegulatory@dfs.ny.gov.
