Non-Resident Licensing Checklist for Business Entities
Download and Fill Out Application
- NAIC Uniform Application for Business Entity Insurance License/Registration
Financial Requirements
- Attach a check or money order for $80 made payable to Idaho Department of Insurance.
Other Requirements
- Officer of the firm must sign and date application.
- One licensed producer must be registered to business license and should be listed on application where appropriate.
- Appointments: The appointing insurer may attach an appointment (INS-LIC-APPT-07) form with the individual's application OR file a notice of appointment within fifteen (15) days from the date the agency contract is executed or the first insurance application is submitted.
- Attach a check or money order for $80 made payable to Idaho Department of Insurance.
- Variable line will require CRD number for FINRA verification of a securities license.
- Adjuster license use NAIC Uniform Application and write ADJUSTER across top front of application.
- Surplus Line: Idaho does not license agencies for Surplus Lines. Agency must carry the P&C license and have registered producer carrying the Surplus Lines license. See Surplus Lines page on DOI website for more information regarding this license type.
- Bail Bond agency please attach bond and appointment from Surety Company.
- MGA: please request MGA application packet from licensing. See below for contact information.
- TPA: please go to the Third Party Administrator section under Licensing Services on DOI website for application forms and information.
- Title Agency: please go to the Title Agent section under Licensing Services on DOI website for application forms and information.

Financial Requirements
- Attach a check or money order for $80 made payable to Idaho Department of Insurance.
Other Requirements

Original test scores and fingerprint receipt from Pearson VUE Testing Services. Pearson VUE website is www.pearsonvue.com; telephone number is 888-204-6218.
NOTE: Letter of Clearance from domicile state can be submitted in place of exam scores if moving to Idaho to become resident producer. You will still need to submit fingerprint receipt.
If there are problems with getting fingerprints done through Pearson VUE, please contact this office ((208) 334-4250) and ask for help from producer licensing.
- If you are requesting the Variable line on your initial license, please include your CRD # on your application. Go to www.finra.org for help with this information.
- Adjuster license: please use the NAIC application and write ADJUSTER across the top front of the application.
