Non-Resident Licensing Checklist for Business Entities

Other Requirements

  • Application Fee is $30, plus $45 License Fee ( Make check payable to the Department of Consumer and Business Services or DCBS)
  • Oregon does not require a designated/responsible licensed producer to be an officer, director, or partner of a business entity.
  • Business entities are required to have a licensed and affiliated agent. Submit Form #2139, Firm or Corporation Affiliation of Agent, Adjuster or Insurance Consultant

Appointment Requirements After Licensing

  • Appointment must be received within 30 working days of effective date of contractual agreement with company.

Non-Resident Licensing Checklist for Individuals

Other Requirements

  • Application Fee is $30, plus $45 License Fee (Make check payable to the Department of Consumer and Business Services or DCBS)
  • Surplus lines applicants must hold a property and casualty license.
  • Variable life applicants must have passed the NASD series 6 or 7 and be registered to an Oregon securities dealer.
  • An affiliation is required if an individual is representing an agency. Form #2139, Firm or Corporation Affiliation of Agent, Adjuster or Insurance Consultant needs to be filed within 30 days of acting on behalf of an agency. http://insurance.oregon.gov/producer/licensing_form-numeric.html

Resident Licensing Checklist for Business Entities

Organizational Documents and Other Requirements

  • Producer license fee is $75. Make check payable to Department of Consumer and Business Services.
  • Surplus lines business entities must also be licensed for property and casualty.
  • Businesses located in Oregon must register their business name with the Secretary of State Corporation Division.
  • Business entities are required to have a licensed and affiliated agent. Submit Form #2139, Firm or Corporation Affiliation of Agent, within 30 days of the effective date of hire.

  • Producer license fee $122.25 payable to the Department of Consumer and Business Service.
  • Fingerprint card, Form #FD258
  • Criminal Records Request form 4862
  • Variable Life applicants must have passed the NASD series 6 or 7 and be registered to an Oregon securities dealer.
  • Surplus lines applicants must also apply for property and casualty, if not already licensed for these.
