Contact and mailing address changes should be completed by the company through State Based Systems. The Department will no longer process changes from UCAA Form 14. Please visit www.statebasedsystems.com and select “Idaho” from the jurisdiction dropdown.

IL Yes Cannot change statutory home office city in Illinois using Form 14.

Company may update available addresses via the Company/Entity Address Portal. Link is located on the IDOI website at IDOI: Company/Entity Financial Compliance under the Resources section. If individual making the updates doesn’t have the required PIN# they may select the “Need PIN?” link to display the current PIN contact. If PIN contact isn’t established or needs to be updated a request can be submitted to fnclsvcs@idoi.in.gov .

NY Yes The Life Bureau prefers  hard copies, especially for large files that are easily readable in hard copy format. The Property Bureau will accept the UCAA submission. The Health Bureau requests that hard copies be mailed to the Health Bureau and email copies be sent to: HealthBureauRegulatory@dfs.ny.gov. All filings should be submitted to the appropriate NYDFS portal which should be indicated on the NYDFS website.
